The home care team walked the final stretch to reach Xolani. At his home, a care worker spoke with him and checked his condition.
Xolani needed further medical help. The nurse agreed to return, speak with his family and prepare transport to a clinic. The newsletter records this plan but does not say what happened after the visit.
The encounter shows why regular home visits matter to people in remote areas: the team listens, provides care where people live and helps them reach further services.
